Motion360 Therapy is an out-of-network provider. After each session, Motion360 will email a receipt to the registered address on file, which includes the amount paid and the date of service. Upon request, Motion360 can provide a superbill for each session, including applicable therapeutic codes that can be used to submit to your insurance provider for potential reimbursement. 

Please note that not all diagnosis codes may be covered by your insurance. It is recommended that you contact your insurance provider in advance to confirm coverage. Once you reach your out-of-network deductible, insurance may reimburse you directly based on their out-of-network rate.
